**Core Responsibilities:**
The following outlines the primary responsibilities associated with the Case Management Program (CMP), encompassing all essential phases and ensuring adherence to established case management (CM) standards of care:
- Actively engage in all aspects of the CMP while ensuring compliance with CM standards of practice.
- Provide specialized nursing insights throughout the CM process, which includes comprehensive assessment, strategic planning, implementation, coordination, and diligent monitoring. Recognize and implement local CM opportunities effectively.
- Formulate and execute localized strategies utilizing diverse CM modalities, including inpatient, outpatient, onsite, and telephonic approaches.
- Develop and implement supportive tools for case management operations, such as patient identification and assessment frameworks, clinical practice guidelines, algorithms, CM software, and databases that catalog community resources.
- Integrate the realms of case management (CM) and utilization management (UM), ensuring synergy between nursing case management and social work case management principles.
- Maintain collaborative relationships with pertinent community agencies and organizations to facilitate comprehensive care.
- Accurately collect and document patient care data to support clinical decisions.
- Design treatment plans that encompass preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions, ensuring continuity of care aligned with optimal wellness objectives.
- Establish and monitor mechanisms that guarantee the effective implementation of patient treatment plans and follow-up processes after discharge in ambulatory and community healthcare environments.
- Provide targeted healthcare education to patients and their caregivers, tailored to identified learning requirements.
- Coordinate patient care collaboratively with a multidisciplinary healthcare team.
- Drive the achievement of optimal patient outcomes related to clinical efficacy, quality, and cost management.
- Ensure compliance with established standards of care and practice, adhering to all pertinent policies, procedures, and guidelines within the Military Treatment Facility (MTF).
- Conduct comprehensive physical examinations and detailed health histories.
- Recognize and promptly address adverse signs and symptoms in urgent situations.
- Communicate effectively and collaborate with a diverse range of stakeholders to keep the healthcare team informed of plans and actions while facilitating teaching and education for patient and family benefit.
- Schedule referral appointments and coordinate specialty care as required.
- Provide nursing services as specified in the Task Order (TO) and engage in research initiatives aimed at enhancing practice effectiveness and patient outcomes.
**Work Schedule:** The established work hours are from 0730 to 1630, Monday through Friday, inclusive of a one-hour uncompensated meal break.
**Minimum Qualifications:**

**Education:** Bachelor of Science in Nursing (BSN) from an ACEN or CCNE accredited institution.
- **Experience:** A minimum of one year of case management experience, particularly with military Active Duty service members. Familiarity with Department of Defense (DoD) healthcare systems, Federal and State healthcare organizations, including DoD Tri-Service systems, TRICARE, and the Department of Veterans Affairs, is essential, alongside a basic understanding of the Recovery Care Coordinator program. -
**Certifications:** - Certified Case Manager (CCM) from the Commission for Case Manager Certification.
- Certified Disability Management Specialist (CDMS).
- Certified Rehabilitation Registered Nurse (CRRN) from the Association of Rehabilitation Nurses.
- Certified Occupational Health Nurse (COHN) or Certified Occupational Health Nurse-Specialist (COHN-S) from the American Board for Occupational Health Nurses.
- Advanced Certification in Continuity of Care (ACCC) from the National Board for Certification in Continuity of Care.
- Certified Rehabilitation Counselor (CRC) from the Commission on Rehabilitation Counselor Certification.
- Nurse Case Manager (RN-NCM) from the American Nurses Credentialing Center.
- Care Manager Certified (CMC) from the National Academy of Certified Care Managers. -
**Licensure:** A current and unrestricted nursing license in one of the fifty states, the District of Columbia, Puerto Rico, Guam, or the U.S. Virgin Islands.

** Current AHA or ARC Basic Life Support (BLS) Healthcare Provider certification.
